Membership terms:

  • Membership does not include gratuity.
  • Services are to be received within the 6-month duration of the contract. No rollover of services post 6-month contract.
  • Membership dues paid on the 1st of the month.
  • Subsequent month dues will be charged to the credit card on file the first banking day of the month.
  • Service cancellations require 48-hour notice via telephone – 832-286-1011. If Service is not canceled at least 48 hours prior, the client will be charged an additional $25 late cancellation fee.
  • If membership is canceled prior to 6 months, a $100 cancellation fee will be charged.
  • Memberships will continue on a month to month term after the initial 6-month term.
  • Membership changes or cancellations require 30-day written notice prior to the next month’s billing period.
  • You may contact Lifespan Euro Med Spa at [email protected] to obtain the cancellation form. $30 bank fee will be charged to the card on file for NSF and a second bank draft will be attempted.
