Membership terms:
- Membership does not include gratuity.
- Services are to be received per month duration of the contract
- Subsequent month dues will be charged to the credit card on file the first banking day of the month.
- Service cancellations require 48-hour notice via email – info@lifespaneuromedspa.com
- If membership is canceled prior to 12 months, a $100 cancellation fee will be charged.
- Membership changes or cancellations require 30-day written notice prior to the next month’s billing period.
- You may contact Lifespan Euro Med Spa at [email protected] to obtain the cancellation form.
$30 bank fee will be charged to the card on file for NSF and a second bank draft will be attempted. All past dues must be paid before renewing your membership.
